Exceptions escalated
Any flag or exception requires operator review and a documented decision. The system does not auto-clear or auto-approve exceptions.
Outcome logged
Decision, reviewer, and reasoning recorded. Stage completion or escalation triggers the next workflow gate.
Human oversight
Every stage requires operator review before the workflow proceeds. Override always available with a required reason field logged in the audit trail.
